Eligibility

To receive medical services from Vitality Weight Loss Center, you must:

• Be at least 18 years of age
• Be a resident of Utah (for telehealth services)
• Complete all required intake forms and medical history
• Meet medical eligibility criteria as determined by our healthcare provider

Payment and Refund Policy

• Services must be paid for at the time of service
• Health insurance typically does not cover services provided at Vitality Weight Loss Center
• We can provide itemized invoices for insurance reimbursement attempts
• There are no refunds for services or products once rendered or dispensed
• Medications cannot be returned once dispensed per state regulations

Appointment Policy

• A $50 fee may apply for late arrivals or missed appointments
• Please provide at least 24 hours notice for cancellations
• Follow-up appointments are required to maintain treatment

Controlled Substances

Certain medications prescribed at Vitality Weight Loss Center may be controlled substances. By receiving these medications, you agree to:

• Take medications only as prescribed
